You can send a questionair to customers by defining a notification just like this:
{$renderQuestionsForMailEmbedded}
This will lead to an email with an embedded form
If you need a different url , you can use {appurl()} as any template
You can also define the HTML content manually in the template
You can get a link using {$questionsURL}
